O’Sullivan went three frames down early on but clawed the tie back to 3-2 before Guoqiang took the sixth frame to progress.
John Higgins booked his spot in the next round after beating Andrew Higginson 4-1.
The Scotsman returns to the table on Wednesday as he faces David Gilbert while Judd Trump goes head-to-head with Matthew Selt.
